Christian Heights at Tenova

Hospital Case Managers are professionals in the hospital setting who ensure that patients are admitted and transitioned to the appropriate level of care, have an effective plan of care and are receiving prescribed treatment, and have an advocate for services and plans needed during and after their stay. The Christian Heights team met with the Tenova Healthcare case management team to discuss ways to streamline the referral and admissions process. Both teams addressed best practices and strategies that could make the process more effective and successful. Discharge planning is important, and both teams will work hard to do what’s in the best interest of the patient.
